If you are a potential new client seeking veterinary care for your pet, we require review of your pet’s complete medical records prior to confirmation of your appointment.  After receipt of your pet’s medical records we will call to confirm or cancel your appointment.  Once your appointment is confirmed, we will email or provide a hard copy (per your request) of our registration forms to you.  Requesting an appointment is not an act of establishing care or a Veterinary Client Patient Relationship (VCPR).  Our priority is to save you the time, trouble and expense up front rather than waste your time, money or delay your pet’s care on a visit when we may not be a good match.

Requesting an appointment is not a guarantee that we will see your pet on the date and time you selected via our online booking portal, text, email, phone, in person, or other method of scheduling.

Urban Pet_Rx may cancel your appointment or deny to see your pet for any reason or for no cited reason at all.  Urban Pet_Rx reserves the right to confirm and schedule your appointment or to reschedule your appointment.
